Once you have reviewed the instructions to proceed to online forms
Please read the following instructions completely prior to leaving this screen to begin the completion of forms.

All Student Athletes:

1. Click on the “Electronic Participation Forms” tab above.

2. Click on the “Sports Participation Packet”.
  • a. Fill in the student information at the top of the screen
  • b. Read and check all forms. Links to all forms have been provided so that you may read them in their entirety.
  • c. Type your student name, and have student electronically sign the form. (Be sure to check the “I agree” box next to the signature box.
  • d. Parent/Guardian complete electronic signature in the same format.
  • e. Provide a return email address near the bottom
  • f. Click “Submit”

  • 3. Click “Go to next form”
  • a. Fill in all information on this form
  • b. Complete Electronic Signature as you did on previous form and Submit.

  • 4.ALL student athletes must download and print the “Pre-Participation Physical Evaluation” form. A new medical history and physical is required every year the athletes plans to participate in athletics.

    5. Fill out the “Medical History ” side of the form. This form must be turned in to the following:
    a. High School Students- Must turn in to Professional Athletic Trainers (do not give this form to any other person at the school. Athletic trainers will not be responsible for forms not handed directly to them by the student or parent athlete).
    b. Middle School Students- Must turn in to appropriate Athletic Coordinator at their middle school. (Again, the coordinators will not be responsible for forms not handed directly to them by the student athlete).

    6. Castleberry ISD requires all athletes planning to participate in athletics to have a new physical every year. Also, if you answered yes to questions 1-6, you MAY need further medical evaluation. It is important that you discuss this with your middle school coordinator or athletic trainer to determine whether you are required to obtain a new physical and turn this in to either the high school coordinator for 8th graders, or the athletic trainer for 10th and 12th graders.

    7. You are required to have a new physical prior to participation in these years. Castleberry ISD athletic department policy requires that the physical be dated April 22nd or later of the year you enter this grade. Unfortunately, this may require some athletes to have a second physical within a short period of time to fill this requirement. This sometimes affects athletes who joined a sport during the spring semester (including but not limited to cheer tryouts).
